Uveitis Workup: The Importance of a Targeted Approach

Dr. Akshay Thomas recently gave a talk on the importance of a targeted approach in uveitis patients.
Evaluating uveitis involves more than ordering a series of laboratory tests. Our uveitis specialist, Dr. Akshay Thomas, explains, a thoughtful diagnostic approach begins with the patient's ocular examination and clinical presentation.
Rather than relying on broad, routine laboratory panels, Dr. Thomas emphasizes selecting tests based on individual findings and the suspected underlying cause. Extensive testing can sometimes produce incidental positive results that may not be related to the patient's eye inflammation.
Letting the Examination Guide Testing
The location and pattern of inflammation help determine which diagnostic tests may be appropriate. Anterior, intermediate, posterior, and panuveitis can each present different diagnostic considerations.
Dr. Thomas discusses several factors that may guide testing, including infectious and inflammatory conditions such as syphilis, sarcoidosis, and tuberculosis. Additional laboratory testing or imaging may be considered when specific examination findings or symptoms warrant further investigation.
Understanding the Results
Selecting the appropriate tests is only part of the process. Laboratory results must also be interpreted within the context of the patient's clinical presentation. A positive result does not necessarily establish the cause of ocular inflammation, just as a negative systemic workup does not rule out eye-specific disease.
Dr. Thomas emphasizes that every test should have a clear purpose, with consideration given to how the results may influence patient management.
Through a systematic, individualized approach, physicians can better evaluate ocular inflammation and make informed decisions about patient care.
